Oversee day-to-day operations at an Arbor hospital unit in either Brooklyn or Queens. Coordinate with physicians and clinic staff to ensure the proper evaluation and treatment of clients. Identify, hire, train, and supervise approximately 15-20 staff. Develop CSPs based on assessment and in conjunction with participants, including clear short- and long-term goals, specific activities, and mutual responsibilities. Effectively interview participants as to needs, goals, interests, and barriers, and appropriately refer participants to activities such as workshops, education, and skills training. Assist participants through counseling, referrals, monitoring, following-up with other agencies, facilitating participant problem-solving and providing intervention services as needed. Maintain and update participant files, ensuring that all case management sessions are documented and all contractually required documents are maintained in files. Ensure that staff conducts outreach activities including attempts to contact participants(s) via telephone, outreach letters and home visits and enter FTR codes into NYCWAY in a timely manner. Monitor and document participant progress towards goals documented on the CSP. Responsible for monitoring and tracking participant’s compliance and attendance. Responsible for submitting weekly reports. Responsible for documenting participant activities. Maintain participant files. Arrange supportive services and complete forms for participants by making phone calls, etc. Performs other duties as assigned. Master’s degree and minimum 5 years’ experience in a clinic setting. Supervisory experience required. Strong management skills. Strong written, oral communication and interpersonal skills. Exceptional organizational skills and strong attention to detail. Excellent skills in Microsoft Office Excel, Access, Outlook, and PowerPoint. Internet experience for research and communication necessary..
